Schedule an Appointment
To schedule an initial appointment at WSCC, please fill out an Intake Form (Child Intake / Adult Intake) and submit it to WSCC. A scheduler will contact you to arrange an initial assessment appointment. Necessary paperwork will need to be completed and submitted at the time of this appointment. A WSCC intake team will determine your needs and match you with a specialist who is most suited for you or your child.
Payment is due at the time services are rendered, unless other arrangements have been made with the Business Services Manager.

Insurance & Billing
For clients with insurance, please bring the following:
- Current insurance card
- A copy of your Authorization Form (if applicable) from your insurance provider. To find out whether or not an authorization is necessary, call the "Mental Health" number located on your insurance card. Bring this form with you to your first appointment or have your provider fax a copy to WSCC at 208.343.0064.
- Business Psychology Associates (BPA), Employee Assistance Program (EAP) and Crime Victim Compensation participants, must bring a copy of the Authorization Form to their first appointment (call your representative for a copy of this form if you do not have one).
For clients with Medicaid, please bring the following:
- Current medical card
- A Healthy Connections Referral Form for yourself or your child, from your Primary Care Physician (PCP). Your PCP can fax this referral to WSCC at 208.343.0064.
To apply for a subsidy, please bring the following:
A completed Subsidy Program Application Form, along with the other necessary paperwork (listed on the application) to verify income. Once the Business Services Manager has received all of your paperwork, she will contact you to discuss eligibility.
